You will be asked to complete this enrolment form and attach the following documentation:
- A copy of your child’s birth certificate or passport.
- A copy of your child’s up to date immunisation schedule from Medicare.

Children all react differently to being away from their parents, and we encourage you to remain with your child for as long as you feel necessary to ensure your child’s well-being.We recommend that you bring your child for a visit of at least an hour, prior to beginning care at the service. This will help to introduce yourself and your child to the educators and other children and to familiarise your child with our setting. Parents and family members are welcome to visit our service at any time.
Your service Coordinator will provide you with a detailed list upon your enrolment.
Please ensure all belongings are clearly labelled with your child’s first and last name.
It’s best for your child, other children and educators to keep your child at home if they are feeling unwell. Please call your service coordinator to let them know of your child’s absence. You will also be required to sign the attendance sheet when you next attend so as we have a record of the absentee day.
Advice to families will depend on the nature of the illness. For example, if a number of children at the same service seem to be showing symptoms of the same illness, the service will let you know what to look out for and if your child should stay at home. If there’s been a confirmed case of an infectious disease in the service, the room the child is in will display an infectious disease fact sheet so that parents are aware of symptoms to look for.
The daily fee depends on the service your child attends, and the room that they are enrolled in. There is a different fee structure for Nursery, Toddler and Preschool age groups. Contact the family relations team on (02) 8362 3000 for more information about the applicable fees.

This is triggered by the Department of Education and Training which we have no control over. We are always happy to answer your questions and are generally up to date with information from the Department of Human Services.
In order to retain your child’s weekly position fees are still payable regardless of public holidays.
In order to retain your child’s weekly position, fees are still payable regardless of absent days. Families are entitled to 42 allowable absences per child per financial year. After these allowable absences are utilised a medical certificate is required for every absent day after the 42 allowable absences. If a medical certificate is not provided, you will not be entitled to any Child Care Subsidy and the full fee will apply.
There is one Subsidy offered by the Australian Government for which you may be eligible: The Child Care Subsidy The Australian Government provides assistance to families to access affordable, high quality child care through the Child Care Subsidy. Three things will determine your family’s level of Child Care Subsidy: Family Income – Combined Activity Test – The activity level of the parent undertaking the least activity Service Type – Centre Based Care Please contact the Family Assistance Office on 13 61 50 and your accountant to confirm details.
The centrepiece of the package is the Child Care Subsidy (CCS). The Child Care Subsidy replaces the Child Care Benefit (CCB) and Child Care Rebate (CCR) with a single, means-tested Subsidy. It will be paid directly to service providers to be passed on to families. It will be simpler than the current multi-payment system and will be better targeted and provide more assistance to low and middle income families. Families earning $66,958^ or less will receive a Subsidy of 85 per cent of the actual fee charged (up to 85 per cent of an hourly fee cap). For family incomes above $66,958^, the Subsidy gradually decreases to 20 per cent when family income reaches $341,248^. For families with incomes of $351,248^ or more, the Subsidy is zero per cent. The Subsidy gradually decreases by 1 per cent for each $3000 of family income.
It is important to note that CCS cannot be claimed prior to your child’s first physical day of care or after your child’s last physical date of attendance at the service. Any absences occurring after the last physical date of attendance until the agreed termination date will be charged at full fee. More information regarding payments can be found at www.familyassist.gov.au.
If you are on an income support payment you may be able to get the Transition to Work Subsidy. This is a payment to help you with the cost of your approved child care by paying some of the “gap fee” up to 95% of the fee charged up to 95% of the hourly rate cap. Please advise the service coordinator upon enrolment if you have been approved for Transition to Work or are planning on applying for the Transition to Work Subsidy.
